At pharmika consulting we nurture a proactive philosophy to the program. We call this unique approach "pharmika inception." Community Pharmacies are in the ideal position to identify patients that are at risk of medication misadventure. Exposure to high risk patients occurs on daily basis. Unfortunately most of these high-risk patients will not have had a HMR. A majority of them, will not even know the service exists. Pharmacists are trained to identify and resolve medication related issues and to ensure the quality use of medicines. By harnessing this inherent expertise, screening and detection of eligible patients becomes as simple as being HMR conscious. Shop staff often develop strong relationships with regular cliental and can also be trained to help facilitate the screening process. By assisting local GPs in identifying patients for HMRs will help strengthen existing relationships and encourage additional growth of the program. Once the right systems are established, integrating HMRs into routine communication with patients and their GPs will become effortless. When acting as an agent of your pharmacy, we will always introduce ourselves to the patient as a member of your staff. Some Community Pharmacists may wish to conduct the interview component of the process themselves. Please contact us for more information or to request an Interview instructions and questionnaire form. After the patient interview has been completed, an accredited pharmacist from pharmika consulting will prepare a HMR report which will include a summary of information obtained from the interview. This report may also contain some recommendations for the GP or instructions for the pharmacy. Once a HMR is completed, the community pharmacy will receive from pharmika consulting; 1. The HMR report form (a copy of this form will have already been sent to the referring GP on your behalf) 2. A signed HMR claim & confirmation form 3. An invoice (please contact us for information about our schedule of fees) Handling completed HMRs: Clinical role of the Community Pharmacy Clinical role of the pharmacy; is to read the HMR report and help deliver on the objectives. Some examples of duties may include;
The HMR Claim and Confirmation form (s) can be sent to Medicare once per month. (This process can be done as an "end of month" task) If multiple HMRs have been completed during that month, the HMR Claim and Confirmation forms should be accumulated and claimed for together. Completed forms will need to be accompanied by a HMR Claim For Payment Coversheet. Copies of these forms are available; For "claim reference number" use the year, month and number of services being claimed. eg If 4 HMRs have been completed during October 2010 you could use reference number 101004. Completed forms can be sent to Medicare; by mail; Community Pharmacy Agreement Officer Pharmaceutical Benefits Section Medicare Australia GPO BOX 9826 Adelaide SA 5001 or fax; 08 8274 9373 Medicare will process claims on the 4th day of each month. Payments are made into the bank account where pharmacy prescriptions are paid, normally within a couple of weeks. Pharmika Consulting does not expect payment for our services until that time.
