Services OverviewOur ServicesMedication Management Reviews (MMRs)Medication management reviews are available in different formats depending on the living arrangement of the at-risk patient; Home Medication Review (HMR)A Home Medication Review (HMR) aims to optimise quality use of medications by facilitating consumer understanding and ensuring judicious use. At pharmika consulting, our extensive experience and our passion for consultant pharmacy ensures a consistently premium HMR service. Detailed information more relevant to each of the participating entities is available on the dedicated pages; Residential Medication Management Review (RMMR)An RMMR is essentially a HMR conducted for a patient that lives in a Government Funded Residential Care setting (eg in an Aged Care Home). Patients in this setting are often less autonomous when it comes to their medications as they are usually administered by nursing staff. The focus of a RMMR is thus more skewed towards rationalising medication use rather than the predominantly educational focus of a HMR. An auxiliary education and quality use of medicine service is also provided to all our Aged Care Home cliental. Pharmika Consulting can be engaged to perform RMMRs in two ways;
Hospital Discharge Reviews The Federal Government has proposed some changes to HMR referral criteria as part of the recently negotiated 5th Federal Government-Community Pharmacy Agreement. The new HMR model may allow patients who have been recently discharged from Hospital to access an urgent HMR that is provided within a short period of time. Discharge staff at hospitals may be granted the authority to generate HMR referrals under specific circumstances. (currently only a patient's regular GP can initiate referrals) As the Business Rules pertaining to this have not yet been written, this potential change remain speculative. We will continue to post up to date information on this topic as it comes to hand.
